Summary In the U.S. and other countries, “nurse practitioners” (also known as nurse examiners) play an important role, performing duties similar to that of physicians. Medical professionals known as nurse practitioners (NPs) offer a variety of primary and specialized medical services, either alone or in conjunction with a doctor. Although having a doctor on every island is challenging, the nurse practitioner's increased power enables more straightforward care and a greater breadth of assistance. A doctor from the mainland only comes to some islands once or twice a week, and only in the morning. The elderly might receive home health care and support from a nurse practitioner on such an island. To provide more convenient medical services, telemedicine is expected to be integrated with unmanned logistics using drones. Drones would deliver issued medications after patients received online medical care and medication guidance. Overseas cases show that it is difficult to have equal medical care on remote islands due to the transportation of supplies, lack of specialist doctors, etc. We wish there were communities like the one in Molokai, Hawaii, where people could help each other with abundant medical care and mental health care, but this is considered difficult in Japan, where the population is aging.
